Hourly rates apply to initial sessions, psychotherapy, in-person or telephone consultations, record review, school visits, report writing, and consultation with other professionals. There is a two-hour minimum for school visits. Our intake appointments are typically 90 minutes. When we are unable to schedule a 90 minute opening, we will often schedule either a 45 or 60 minute session. Most therapy sessions are 45 minutes, although we frequently see clients for 60 minutes, depending on need and availability.
Payment for Services
Payment is expected at the time of service unless other arrangements have been made in advance with your service provider.
Payment can be made by check, cash, or credit card (Visa, MasterCard, Discover, or American Express).
Checks should be made out to the ADHD Performance Counseling with a notation on your check of the name of the associate who has provided the service.
We ask all clients, including those who prefer to pay by check or cash, to provide us with a valid credit card number so that in the event that payment is not made by check or cash at the time of service we can apply the appropriate charge to your credit card. For any account that has an outstanding balance at the end of each month, that balance will be charged to your credit card account.
Dr. Shane K. Perrault’s Fees
- 45 minutes - $160.
- 60 minutes - $195.

ADHD Performance Counseling Associate's Fees
- 45 minutes - $125.
- 60 minutes - $160.
- 90 minute consult - $210.
Psychological Test Battery
- Intake interview (90 minutes) - $250.
- Testing/Scoring/Verbal Interpretation $1,100.
- Standard testing includes, CPT, BASC-2, BRIEF and Multiple Intelligence Test. (These assessments evaluate Attention, Behavioral and Psychological Executive functioning, and Learning Style).
- Report writing - $750.
- Feedback session (90 minutes) - $250.
Total Evaluation cost –
- Evaluation Report A: $1,600, includes computer generated reports, verbal summary and intervention strategy session.
- Evaluation Report B; $2,100, includes written report and computer generated reports, verbal summary and intervention strategy session).
The testing fees listed above are estimates based on the average amount of time required for a psycho-educational test battery. If additional testing is necessary, then additional charges will apply at Dr. Perrault's or at the Associate's hourly rate. Additional fees with be discussed prior to the additional testing and charge.

During the course of treatment, it may become necessary to increase fees. Fees will be reviewed periodically and will be increased no more than once during any calendar year.
The Center does not have a sliding fee scale; however, each associate, at her / his own discretion, may agree to reduced fees under certain circumstances, such as sudden unemployment, loss of health insurance, etc., that occur during the course of treatment. Such fee reductions should be documented in writing with a signed agreement kept in your records to avoid misunderstanding about fees and payment schedule.
Counseling / psychotherapy appointments must be cancelled more than 48 hours in advance, except in the case of true emergency. Our full, standard fee will be charged for last minute cancellations that are not due to a true emergency. In the unlikely event that we are able to fill this time with another appointment, you will not be charged for a last minute cancellation.
Testing appointments must be canceled at least 5 business days in advance, except in the case of true emergencies. Testing cancellations made less than 5 days in advance will result in a charge of $400. Our psychologists set aside many hours in their schedule to accommodate testing, and this fee only partially reimburses them for their time.
Insurance Coverage for Services
ADHD Performance Counseling does not accept direct insurance payment for services and does not participate in any insurance provider network.
Our services are covered by most health insurance plans that cover out-of-network providers. It is the responsibility of the client to communicate directly with his or her insurance company regarding coverage or reimbursement issues.
Statements will be emailed on a monthly basis; on or about the tenth of each month, covering all activity for the preceding month. These statements are designed for submission to insurance companies, reflecting all charges, payments, diagnostic codes, and procedure codes. Statements will be e-mailed to the address provided, unless you request otherwise.
