What to Bring to Your First Appointment

  • Credit card: we accept payments by cash, check or charge. We keep credit card information on file for each client in the event there is an outstanding balance.
  • Copies of records (Medical/Psychological/School/Employment) Report cards, test scores, test reports or any other records pertinent to ADHD or learning disabilities are very helpful to us in making an accurate assessment. Please only bring photocopies of these reports rather than original copies. We want to guard against the possibility of losing your original copies.

Preparing for First Appointment (After You Have Scheduled an Appointment)

To streamline your first contact with our clinic, please print the following forms.

  • Client Information Sheet: you must FAX this form to us in order to confirm your appointment.
  • Center Policies and Procedures (for your records): This document includes fees and payment information, which can also be found here.
  • For clients under 18: Child Intake Questionnaire.
  • For clients 18 and over: Adult Intake Questionnaire.

If you would like us to discuss your appointment with anyone else (e.g. family member or physician), please also fill out the:

  • Consent for Exchange of Information form.

If you prefer to complete the Intake forms at the clinic, please be sure to arrive at least 20 minutes in advance of your appointment time.

What Happens During Your First Appointment

Depending upon the age of the child we may see the parents alone, the child alone, or may divide the first assessment session between parents and child. Your first appointment will typically be a 90 minute clinical interview, during which we will ask you many questions about your current concerns, as well as about your child’s developmental, academic and psychological history. We will also ask about family history as it is relevant to your child.

Completing the intake questionnaire and sending it to us in advance of your initial appointment will facilitate the assessment process, allowing us to develop recommendations and begin treatment with your child right away.

The first several sessions may focus on information gathering and assessment, depending upon the individual circumstances of the child. Most often, the ADHD Performance Counseling staff member you consult with will recommend a neuropsychological or psycho-educational assessment. In some cases, the staff member you consult with will also administer some or all of the tests. Further testing may not be necessary if your child has been recently tested.

The first consultation will be a 90-minute clinical interview in which we will ask you about your current concerns, your reason for scheduling a consultation, and your family, educational and employment history. In many cases it is necessary to continue this interview process for a second appointment, which would be scheduled at the time of your first visit.

Often, we will ask you to complete questionnaires following your initial consultation. We will review your responses to these questionnaires and discuss them with you at your second appointment.

Testing is not automatically part of the assessment process for ADHD in adults. Diagnosis of ADHD is typically made through an extended clinical interview and the completion of ADHD questionnaires. If we believe that testing is desirable, such as in the case of suspected learning disabilities (which very commonly co-occur with ADHD) or when there are questions about diagnosis or co-existing conditions, we will discuss that with you during your initial clinical interview.
